Frequently Asked Mobility Questions

You will need to obtain a prescription from your provider for the requested mobility device, once obtained it can be faxed to 402-408-1085 or dropped off at Kohll's Rx in Millard. Along with the prescription, insurance information will need to be obtained in order for us to follow the correct steps as each insurance is different when it comes to their requirements. Once this is obtained by the mobility team you will be contacted for further information.

To get insurance to cover your mobility equipment, your insurance provider must determine that you meet their criteria for”medical necessity” of the requested equipment. This means that you must have a condition that requires the equipment to prevent, diagnose, or treat an illness, injury, contrition, disease, or its symptoms. Coverage varies on the policy that you hold, that policy will determine if it is 100% covered or if you will owe a percentage of the equipment. Most equipment is covered by insurance plans when the appropriate documentation is submitted and approved.

Kohll's Rx accepts most major funding sources and health plans. Kohll's Rx is responsible for contacting the health plan to determine if we are in network with your plan.

Please know that due to insurance requirements, the process to obtain mobility equipment can take time. The time frame can take 2-3 months taking into consideration all the documentation that has to be collected from your physician, therapist, and the time the insurance company takes to review the documentation that was submitted for authorization. This is a rough estimated time frame and can vary based on the factors that are out of Kohll's Pharmacy control. For basic standard wheelchairs the time frame can take up to a month, as insurance companies do not require as much documentation for a basic standard wheelchair. All insurance companies require documentation to determine coverage, there are no exceptions.

No, we can not dispense the mobility equipment until all documentation has been completed and approved in order for us to be paid.

If you are going through insurance and would like to upgrade, you will need to speak to one of our mobility team members for additional information. If you would like to purchase a mobility device out of pocket, you can stop in anytime at our Millard location to do so.

Kohll's Rx will assist with any future needs or services to your equipment. If the item was paid out of pocket, any service done to the equipment will be an out of pocket cost. If the item was covered by your insurance, documentation will have to be collected and an approval will need to be obtained by the insurance in order to complete any repairs or services to your equipment. Insurance Information

Insurance requirements and steps for custom manual wheelchairs, power scooters, and power wheelchairs.

  1. Obtain a prescription from your provider for the requested equipment. The prescription must include the item, diagnosis, length of need, and signature of the prescriber.
  2. ALL insurance companies require the patient to complete a seating/mobility assessment by an OT/PT. Kohll's Rx will help assign an OT/PT to your case if you currently do not see an OT/PT. There are no exceptions on skipping this step. If this step is not completed, the request is automatically denied by insurance companies.
  3. A face-to-face appointment with the provider is needed in order to discuss the findings of the evaluation, and for the provider to provide office notes to discuss the requested device. During this appointment the doctor will review and sign the assessment from the OT/PT, provide office notes, provide an updated prescription if needed, complete a 7-element form, and complete the detailed product description from. Once completed they will fax the paperwork to 402-408-1085.
  4. Once the paperwork is completed, it will be sent to the insurance company for prior authorization in which a review and decision making will attempt.
  5. If approved, you will be contacted and advised of the approval. You will be advised of how the insurance will pay for the device, how much they will pay, and if you will owe any portion of the cost. The item will then be ordered if not already on the floor, and then scheduled for delivery once the item is ordered. Please keep in mind any custom device cannot be ordered until there is an approval due to the device being custom to each individual. Any item ordered can take up to 2-3 weeks to arrive at the store.
  6. If denied, you will be contacted with options to appeal the denial or to pay out of pocket for the device.
