Patient CPAP Eligibility Requirements
Criteria for Coverage
Need a prescription, chart notes, sleep study, and (sometimes) an authorization.Prescription / Standing Order
- Your doctor will send over a prescription for a CPAP machine.
Sleep Study
- In order to confirm that you have sleep apnea, your doctor will have you undergo a sleep study test in order to determine the number of episodes where you stop breathing during sleep. This test can be done at a sleep lab, or in your own home.
Chart Notes
- These are the notes that your doctor makes during your visit in order to document your sleep apnea condition. These notes are faxed over to Kohll's Rx.
Once you have all 3 documents (prescription, chart notes and sleep study), we will verify your insurance and calculate your payments if any, and call you to set up your new CPAP machine.
